Admission to hospital is a ‘teachable moment’ in which most patients may be more likely to accept treatment and support for tobacco dependence or are at least willing to attempt temporary abstinence during the hospital admission, which may itself lead to a future goal of long-term abstinence. We work in close partnership with our dedicated community stop smoking services across our localities. Together we have implemented a robust pathway to ensure the transfer of care for our patients from hospital to home is efficient and streamlined, helping to keep our patients on their smokefree journey.
